An Assisted Living Facility is a building that has at least five (5) separate living units where individualized support services are made available to individuals to accommodate needs and inabilities to enable individuals to remain independent. An entity providing assisted living services to five (5) or more individuals in this type of setting must be licensed as assisted living facility by both the North Dakota Department of Human Services and the North Dakota Department of Health.

Facility locations were originally plotted by geocoding completed by Jon Tonneson, formerly of the ND Department of Emergency Services. Geocoded locations were then verified by ND Department of Health personnel by comparing the points with aerial imagery and road map as background layers. If neccesary, the facility in question was contacted to verify their location. If location was deemed not accurate, the point's coordinates were edited to reflect the real location. Elevations were collected using the extract values to points tool in Spatial Analyst. Most recent and detailed elevation source was used to capture elevation at the points. ELE_SOURCE lists the elevation source used. Some facilities had multiple sources of elevation due to more detailed elevation information collected in a particular area.
